If you wish to request a repeat prescription for your pet, please fill in the form below with your details. We will attempt to fulfil your request within 24 hours.
It will only be possible for us to dispense your medication if you fulfil our normal criteria:
- You and your pet will need to be registered with us
- We will need to have seen your pet for a medical examination within the last six months
- The prescription must not have changed since the last repeat, unless a consultation has taken place with the vet
Should the vet have any queries, it is important that we are able to contact you to discuss the situation, so please make sure you provide a telephone number that we can reach you on.
Please call your practice before coming to pick the medication up in case of any unforeseen delays.
